CLAIM APPLICATION FORM / ON-LINE

1. Filling this application form is not equivalent to signing a contract. By completing and submitting this form, the Creditor shall provide the analysis query about the possibility of debt recovery of the claim mentioned in the application form.
2. If EW COLLECTIONS accepts the case, the Creditor receives the General Contract of Services of International Debt Collection with the specific conditions of cooperation. After receiving the Contract duely signed by the Creditor, EW COLLECTIONS Ltd. shall send an acknowledgment of the case to the Creditor .
3. The Creditor states that the claim against the debtor stated in the form is undisputed and due. 
4. The creditor shall authorise in separate written Proxy the company EW COLLECTIONS Ltd. and its partner entities to carry out the debt collection proceedings, including accepting payments from the debtors.
5. The Creditor agrees to the processing of its or his data for marketing purposes.
